ADMISSION HOSPITAL POLICIES

  1. Hospital Visitation and Watcher's Policy
    1. No visitors are allowed during pandemic.
    2. Number of stay in watchers.
      • Only one (1) watcher is allowed and will be given a watcher's pass.
      • Only immediate family member of the patient is permitted to stay as watcher of the patient.
  2. Room and Board Cut off time
    1. Discharge room and board cut off time is 12:00 noon. All patients discharged after cut off time will be charged additional one day. Less than 24 hours confinement will be charged equivalent to one day room and board.
  3. Bringing of items inside the hospital
    1. There are items/equipment/tools/gadgets that are not allowed to be brought inside the hospital premises. These items include: electrical and medical equipment (i.e.: glucometer, nebulizer, pulse oximeter), mattress, folding bed and chair.
    2. Personal computers like laptop, notebook, i-pads, shall be allowed into the rooms once a gate pass has been issued by the security guard. A reasonable amount will be charged daily for electrical consumption.
  4. Valuables

    I understand that Tarlac Medical Center does not assume the responsibility for any loss or damage of personal property and money/jewelries kept in the patient's room during hise/her confinement.

  5. Smoking Ban

    Tarlac Medical Center is a medical institution that adheres to Executive Order 26. Penalties of civil fine shall be imposed for any violation.

  6. Infection Control
    • Use of Personal Protective Equipment (PPE): disposable mask, face shield should be worn all the time.
    • Hospital waste segregation shall be followed by all watchers and visitors.
      • BLACK for non-biodegradable dry waste
      • GREEN for biodegradable wastes (kitchen waste, food scraps)
      • YELLOW for wet waste with possible human discharges (face masks, used tissues, diapers, and pads).
  7. Hospital Payments/Medical Benefits
    1. Tarlac Medical Center adheres to Anti-Hospital Deposit Law Republic Act 109321 Revised Republic Act 8344 "that in emergency or serious causes, it shall not request, solicit, demand or accept any deposit or any other form of advance payment as prerequisite for administering basic emergency care, for confinement or medical treatment, or to refuse to administer medical treatment and support to any patient."
    2. Progress Billing
      A progress bill will be issued regularly including notice for required payment. Failure to provide such amount will lead to suspension of all hospital charges requested for the patient to be charged to his/her hospitalization bill due to deficient payment and accumulation of charges. All drugs, laboratory tests, diagnostic and other procedure shall be paid in cash basis.
    3. Operating Room Clearance
      Patients who shall undergo elective surgical operation shall be required to pay partially an amount depending on the surgical case to be performed at least twelve hours (12 hrs.) prior to the scheduled surgery.
    4. HMO Members
      Patients who are under company account should present their HMO card and must submit the Letter Of Authorization (LOA) before discharge.
    5. Patients with PhilHealth benefits and those with Senior Citizen's card must submit all required documents to the PhilHealth section before the discharge date in order to avail the benefits.
  8. Data Privacy Act

    8.1 In compliance with R.A 10173 - Data Privacy Act of 2012, and for confidentiality of our patients, data subjects and security of data processing, Tarlac Medical Center strictly implements: Taking photos, Audio or Video Recording are not allowed.

Obligations of the Patient

  11. Have adequate health information and actively participate in the treatment
  12. Respect the right to privacy of health care workers and institution
  13. Exercise fidelity on privilege communication
  14. Not to force physician to treat him/her
  15. Respect physician’s decision on medical reasons on his right to religious belief
  16. Obligation to medical records
  17. Participate in the training of competent future physicians
  18. Obligation to inform health care worker to any perceived or alleged violation of his rights by the health worker or institution
  19. Use due process and exhaust grievance mechanism
